The defined patient pathways are based on national and the ICHOM standard. The number of patient pathways within Cardiology is constantly being expanded.

Available Patient Pathways:

Coronary Heart Disease
• Coronary bypass surgery (CABG)
• Percutaneous coronary intervention (PCI)
• Conservative treatment

Heart Failure - Aortic valve
• Aortic valve replacement (AVR)
• Transcatheter aortic valve implantation (TAVI)
• Conservative treatment
Combined coronary and aortic valve failure

Heart Failure - Mitral valve
• Surgical treatment for mitral valve disorders

Heart Failure - Atrial fibrillation

• Catheter pulmonary vein insulation
• Minimally invasive surgical pulmonary vein insulation

Medoc Cardio is a web-based platform for optimally implementing Value-Based Healthcare (VBHC) for Cardiology.

ICHOM dataset

The general, prior history, treatment and outcome variables from the ICHOM standard are included in clinical forms. Care providers can manually fill in these variables. It is also possible to automatically import data recorded in the HIS/EPR via a link.

The PROMs questionnaires are based on the standard set by ICHOM for the defined care paths. Medoc Cardio contains an extensive library of validated questionnaires, from which a specific care path can be chosen.

PREMs (Patient Reported Experience Measures) are questions about patient experiences with regard to his/her degree of satisfaction with treatment, participation, aftercare and information. Patients answer these questions anonymously. Care institutions periodically receive reports on patient experiences.

Implant registration

Fully digital implant registration based on the article number and lot number is possible. Implants can be registered on the basis of a bar code. Linking the hospital information system also allows these data to be imported from the HIS/EPR. The implant registration is part of this care path.

Link to HIS

The link to the hospital information system can be established to obtain patient information and related operation information. Linking the hospital information system offers the advantage of not having to enter data twice. This also better guarantees the reliability of the accuracy of the data.

The VBHC platform was designed in such a way that, if desired, benchmarking can take place between the care institutions that desire it. It is also possible to report on the basis of multiple care institutions, so that there is a larger patient population.

Export to national registry

SDS Medical can export both the PROMs and the implant registration information to the national registries

Export to ichom

SDS Medical can  exports outcome data to ICHOM benchmark projects.
